- Title of Session
- Submitter's Email Address
- Submitter's Full Name
- Name of Group Hosting this Function (if applicable)
- Summary Statement of the Goal or Concept and Objectives: List the goals, concepts, and objectives of the proposed session. Note how this session addresses the theme of the Annual Meeting and the needs of attendees.
- Lunch and Learn Description (50 words or less, required, will be used in Program Book)
- Audiovisual Needs: If you have no Audiovisual Needs, please enter "N/A" into the box below. Please note that ESA cannot make audiovisual (AV) equipment available for all meeting functions. If AV equipment is required for committee meetings, small general meetings, and receptions, organizers have the option to order and pay for equipment or services directly with the AV supplier.
- Catering Requirements: If you have no Catering Requirements, please enter "N/A" into the box below. Please note:
- You will receive a confirmation email indicating the date, time, and location for the food function when it has been scheduled. In addition, you will receive a list of catering contacts. It is your responsibility to order your food and beverage (F&B) directly with the catering contact where your event has been scheduled.
- Per ESA's contract with the Convention Center and hotels, no F&B is allowed to be brought into the convention center or hotels. All F&B must be arranged though the respective facility. Lack of strict adherence to this regulation may result in the facility's confiscation of all F&B not purchased through the facility's caterer. ESA appreciates your cooperation.

- Be sure to add the names of all contributing organizers and any panelists, if applicable, here. Information listed on this page will be used for all notifications and when compiling the program book.
- Before adding a new name, please search the database to see if that name has already been entered.
- If you have been a participant at an ESA Annual Meeting, your information should already be in the database.
- When searching for a name, keep in mind that it returns exact matches only unless you have selected Match "Like".
- While you are given the ability to search by First Name, Last Name and Email Address, it is recommended searching by only one field at a time.
- If the name you are searching for is in our database, but the contact or affiliation information is incorrect, please update the information on record and do not create a new one.
- If you need to enter a new name, please complete as many of the fields as possible.
- Only move on to the next step after all contributing organizers and panelists have been added.
